Have you tried using the optical output in conjunction with the HDMI? In theory it shouldn't help matters, but if you're getting video and not audio, it's possible that switch might only be passing through HDMI video.

Is there an option to define the default audio device? I know on Windows machines that if a video card has HDMI it doesn't always know to use the HDMI port for default audio.

Try www.amazon.com/gp/product/B002WBOQPU/ref=oss_produ.... Less expensive and someone in one of the reviews indicated that it works with the ATV2. I ordered one myself.
