The silicone tips (or caps, or whatever you want to call them) these headphones come with are great to start and have a very snug fit on the buds themselves, but damned if they don't tend to fall off repeatedly after use.
Couple this with the fact that you can't buy new caps (see: discuss.gdgt.com/apple/in-ear-headphones-with-remo... and discuss.gdgt.com/apple/in-ear-headphones-with-remo... ) and you've got a recipe for some suck. That's all -- and fwiw, otherwise, I really like these headphones (see also: ryanblock.com/2008/12/apples-new-in-ear-headphones... ).

Silicone tips keep popping off (and driving me nuts)
I too had problems with the silicon tips falling off. I went out on a limb and ordered some foam replacement tips from www.complyfoam.com/ They are AMAZING! The fit is super comfortable, they are much more firmly attached to the driver case and the sound isolation is unbelievable. A 5 pack of any size if about 20 bucks shipped or you can get pack with one set in each of the 3 sizes. I'd recommend these tips to anyone.
I don't have these, but it seems like the Shure tips should work given the apparent bore size.

The Sony hybrid tips (and some others) are recommended replacements on Head-Fi, I'm probably going to give them a try as mine are starting to become loose (not in the ear, just between the tips and the IEM itself).

My ear buds fit snugly on the screw caps. I know that at the end of last year apple quietly updated these in-ear headphones. The 3.5 mm jack is now made of hard white plastic so I also wonder if they made the ear buds a bit tighter.
Mine fell of so many times I would loose them & had to use all three sizes until they were all gone. When I got a second pair (they have the best sound & I like the controls), I put a little rubber cement between the rubber buds & the earphones themselves. Will see how long that lasts.
