These computers can't read it either.
I was just wondering if this was common, and if you think there is something wrong with the phone itself or a way to test it. I don't want to get a replacement and have that die too.
Also, any tips for salvaging the data on the card or at least getting it to format?

It seems the stock 16 GB micro SD card in my Droid X died. The phone can't read it. I can't format it on my phone, my MacBook, or my Windows PC.
Your SD card might still be covered by a warranty by the manufacturer and can be replaced. Here's the contact information based on your type of card:
- If the phone has a SanDisk card, call SanDisk directly at 866-726-3475 or email: support@sandisk.com
- If it is not a SanDisk card, but you have a Motorola device, call Android support at 800-734-5870.

I actually had this happen to me when I first bought my phone, about 2 weeks in, I took it into the Verizon store, they didn't even say much, just took it went in the back room, said "yup, it's dead" and handed me a replacement. Either this happens a lot or they just gave me a free one because I had just purchased it, either way, take it into a Verizon and see if they might just replace it? I haven't had any problems with the second card since then.
