Currently traveling through central California via train:
On me, I have a Virgin Mobile Novatel Mifi 2200 (which piggy backs on Sprint's network) and my jailbroken iPhone 4 (obviously using AT&T's somewhat notorious service).
I've been working via my laptop all morning and afternoon, and some things I've noticed:
1. Outside of the Bay Area, AT&T coverage is as good, if not better than Sprint.
2. Outside of the Bay Area, AT&T speeds are DAMN fast. I've stopped using the MiFi for my internet access because I was getting 6000ms pings to Google.com -- super slow! I turned on hotspot sharing through my iPhone 4 and am getting 100ms pings and relatively fast downloads. It's pretty slick!
3. In San Francisco proper, the MiFi actually supplements my AT&T service pretty well. In areas that lack AT&T service, my MiFi can find a Sprint signal in most cases (there's been a few instances where both are dead, but it's few and far between).
4. Sprint download speeds in San Francisco seem to be a lot faster than AT&T -- (Example of some AT&T 3G speeds in downtown San Francisco last week, with 5 bars! yfrog.com/5xz9rnj )

Welcome to outside of the Bay Area... I just keep waiting for AT&T to get their shit together there as they have in many other parts of the country, it's as if they enjoy torturing y'all. Austin, TX isn't so bad, AT&T still doesn't understand that SXSW and UT football games demand micro-towers, but the surrounding areas are pretty well plastered with their 3g.
In southern Cali, I regularly get 3Mbps on my iPhone 4 with AT&T. They still over-charge for their service but it has noticeably improved over the last couple of years. Part of the problem in SF is the city permitting process to add more mobile towers. It takes years between proposal and implementation. No mobile company would be able to keep up with big upticks in demand.
